Passports for Children: Under 16 years of age

Valid for 03 Years

Application for the first time applicants/ Application in lieu of Lost Passports

Required Documents

1. Original Birth Certificate (please note that the English translations are not accepted as original Birth Certificate)

2. Citizenship Certificate (if born outside Sri Lanka)

3. Proof of Applicant’s status in Canada: Copy of the Permanent Resident Card (PR)/ Work Permit/Study Permit / Visa/ Letter from Canadian Immigration which indicates applicant’s present status in Canada)

4. Parents Passports

5. Parents’ Consent Letter (with authentication of parents’ signatures)

6. Fee (A money order / Bank Draft for C$170.00  in favour of “High Commission of Sri Lanka” if you wish to get the passport by mail, you may pay additional CA$ 16/- as postal charges or should send prepaid self addressed envelop

7. Three Passport size photographs (3.5 cm X 4.5 cm) Colour, matt finish (gloss or semi gloss photographs will not be accepted), face of the photograph should be straight, both ears should be visible and the photographs should be taken within 3 months of the application.

Additional Documents:

If, a Dual Citizen, the original Dual Citizenship Certificate and a copy of the foreign passport

If, one parent has died,   the death certificate

If, parents are divorced, the court order indicating the legal custody of the child

If, an adopted child, Court order in respect of the adoption of the child

Instructions for completing the application forms:

  • Application should be filled in Block letters
  • Signature of the applicant should be placed within the cage and should not touch the borders of the cage.
  • In case of a child, who is unable to sign, one of the parent should write part of the child’s name in the relevant Signature Cage. Please do not write the parents name or Signature
  • Photographs should be affixed to the application
  • In case of first time applicant / Application in lieu of a lost passport, the declaration by the applicant, the applicant’s photographs and signatures in page No. “02” should be attested by a Notary, Medical Practitioner, Lawyer, Civil Servant, Chief incumbent of a temple, Professor of a University or a person of similar status living in Canada.
  • In case of a lost of a valid passport, police complaint should be attached
